The intake/assessment process can take anywhere from 2-5 hours depending on service needs.
This process may include screenings/assessments with a counselor, a nurse, and a physician.
Engage in Care Planning
Complete an Intake/Assessment
You will develop a care plan with your physician and/or your counselor based upon your needs. This is a collaborative process where you decide your goals for care and identify which services you will be participating in.
Your recommended level of care is guided by the information gathered during your intake/assessment and services will be recommended accordingly.
